TRACK REMINDER

Parents you must turn in the following by next practice if your child is interested in participating in the track program.

*Completed Athletic Participation contract

*Emergency Medical Authorization form

*Doctor's Note stating your child is physically able to participate in program

*$25.00 check made out to St. Michael School

If you need any of the forms, they are available in the office

Please make sure your child is dressed appropriately for track practice and has a water bottle.  Practice ends at 4:00pm, please pick children up at school playground.
